How to Register with this Practice

If you have recently moved into the Lymington area and you would like to register for NHS services at Chawton House please call into the surgery and speak to our Reception Staff who will be pleased to help you.

You will need to bring your Medical Card if you have one and be prepared to provide full details of your home address. We will provide you with a simple application form that you will need to complete to enable us to make a provisional registration on our Clinical System.

We will forward your Medical Card and other important information to our local Patient and Practitioner Service Agency who will ensure that your Medical Records are transferred from your previous GP to us.

In the meantime we request that you attend the surgery for a New Patient Medical Check to enable you to meet with one of our clinicians who will take details of your past medical history. We cannot insist that you attend this check up, however, you will appreciate that if you have any on-going medical problems it is important that we have a record of these as soon as possible. It can take between four and eight weeks for your Medical Records to be transferred.
